    Chloroquine has been extensively used in mass drug administrations, which may have contributed to the emergence and spread of resistance. It is recommended to check if chloroquine is still effective in the region prior to using it.

    Both adults and children should take one dose of chloroquine per week starting at least 1 week before. traveling to the area where malaria transmission occurs. They should take one dose per week while there, and for 4 consecutive weeks after leaving. The weekly dosage for adults is 300mg base 500mg salt. Chloroquine resistance is widespread. Chloroquine should not be used for treatment of P. falciparum infections from areas of chloroquine resistance or malaria occurring in patients where chloroquine prophylaxis has failed. Patients infected with a resistant strains of plasmodia should be treated with another antimalarial drug. Before using chloroquine for prophylaxis, it should be ascertained whether chloroquine is appropriate for use in the region to be visited by the traveler. Chloroquine should not be used for treatment of P. falciparum infections acquired in areas of chloroquine resistance or malaria occurring in patients where chloroquine prophylaxis has failed.

    The Centers for Disease Control and Prevention recommend against treatment of malaria with chloroquine alone due to more effective combinations. In areas where resistance is present, other antimalarials, such as mefloquine or atovaquone, may be used instead.

  6. For travel to areas where chloroquine resistance is present, mefloquine is the only medication recommended for malaria prophylaxis during pregnancy. Studies of mefloquine use during pregnancy have found no indication of adverse effects on the fetus. Experts are evaluating the safety of atovaquone-proguanil use during pregnancy.
